2024-2025 Season Baie d'Urfé Curling Club
I hereby apply for membership:
• I agree to pay the annual fee and all renewal fees for my membership category prior to my first curling game.
• I agree to respect the Club constitution, by-laws, rules and regulations until I resign in writing.
• I agree to pay all monthly statements upon receipt and any unpaid balances will be subject to a monthly
surcharge.
• I authorize the Club to contact me or send me communications by e-mail.
• I grant permission to the Club to use my name and any photograph in which I appear, that is taken in the course
of Club activities, on the Club's website or in media releases.
• I authorize the Club to collect and disclose my personal information, but only insofar as it is used to manage my
curling relationships with the Club, its members and CURLING QUEBEC.
• I hereby release the Town of Baie d'Urfé, the BDCC and each of its members, officers and employees from any and
all claims for damage and cause of action which may directly or indirectly result from my attendance at or
participation in a BDCC activity.
